Nutrition for Wheelchair Users
Wheelchair users generally have different mobility and calorie requirements than able-bodied individuals due to physiological changes following injury or disability, such as muscle atrophy and reduced energy expenditure.
Calorie Needs
Adults who use wheelchairs typically require fewer calories than non-disabled individuals of the same age, height, and weight. While the average man and woman need approximately 2,500 and 2,000 calories respectively to maintain weight, wheelchair users likely need less for several reasons:
- Reduced Muscle Mass:Because wheelchair users often do not use large leg muscles, they have less lean muscle mass, which means fewer calories are needed to maintain their weight.
- Lower Resting Metabolic Rate (RMR):Alterations in body composition, specifically increased fat mass and decreased fat-free mass, lead to a reduction in the resting metabolic rate.
- Lower Physical Activity:Mobility impairments often result in fewer calories burned through daily physical activity.
Specific Caloric Estimates:
- General Assessment:Actual energy needs for those with spinal cord injuries (SCI) are often at least 10% below predicted needs for able-bodied individuals.
- Rehabilitation Phase:During this phase, guidelines suggest estimating needs at 22.7 kcal per kg for individuals with quadriplegia and 27.9 kcal per kg for those with paraplegia.
- Weight Management:When aiming for weight loss, wheelchair users must be cautious because eating fewer calories makes it more challenging to obtain enough essential nutrients, vitamins, and minerals.
Factors that Increase Calorie and Nutrient Needs
Certain conditions can significantly increase a wheelchair user’s caloric or nutritional requirements:
- Pressure Ulcers:Healing a pressure ulcer requires significantly more energy—estimated at 30 to 40 kcal per kg of body weight per day.
- Type of Mobility Device:The use of a manual standard wheelchair increases energy needs, heart rate, and oxygen consumption compared to using ultralight or power-assisted wheelchairs.
- Protein for Healing:High protein intake is crucial for keeping skin healthy and healing existing wounds.
Mobility and Physical Activity
Mobility for wheelchair users often relies heavily on the upper body and shoulders, which can lead to increased stress on those joints. To maintain health, users are encouraged to engage in:
- Aerobic Activity:At least 150 minutes of moderate-intensity aerobic activity per week is recommended to improve heart health and aid in weight loss.
- Adapted Sports:Options include wheelchair basketball, swimming, rowing machines adapted for wheelchair use, and sports like boccia or wheelchair rugby.
- Strength and Balance:Exercises targeting muscle strength should be performed at least two days a week.
Other Nutritional Differences
- Bone Health:Because they may not be weight-bearing, wheelchair users are at a high risk for osteoporosis. Increased intake of calcium and vitamin D3 is often necessary to maintain bone mass.
- Hydration:Proper hydration is critical, especially for those who use catheters, as they are at a higher risk for urinary tract infections (UTIs).
- Bowel Management:Neurogenic bowel dysfunction is common, requiring a diet high in fiber (initially 15g, increasing to 30g) and adequate fluids to prevent chronic constipation.
